Pathways: Waiver Services

   
Information provided by: Chemung County Department of Mental Hygiene
Provides alternatives to traditional programs based on needs, interests and desires of each individual. Options available include: service coordination; residential habilitation; day habilitation; respite; environmental modifications; and adaptive. There is no age limit to services. Individuals must be diagnosed developmentally disabled and must meet HCBS Waiver eligibility requirements. Waiver Services for individuals with Traumatic Brain Injury requires an age of onset of 22 years or older and the individual must meet DOH-HCBS Waiver TBI eligibility requirements. The referral process includes confirmation of eligibility criteria, tour, intake and individualized Service Plan or equivalent.
